Unfortunately, many of the services patients value most — like quick responses, smooth referral coordination, and a stronger focus on prevention and wellness — are not covered by insurance. To continue delivering the level of care you deserve, we’ve introduced a membership program that directly supports these essential services.

With three tiers to choose from, you can select the level of care that best fits your needs: Basic (Free) Membership, offering comprehensive, reliable medical care; Enhanced Membership Package, adding personalized support with dedicated coordinators and enhanced wellness exams; and Concierge Membership, giving you direct provider access and advanced programs in longevity, regenerative medicine, nutrition, and more.

No matter which tier you choose, your membership ensures you receive the compassionate, personalized, and high-quality care that sets our practice apart.

Basic Membership

Comprehensive, reliable access to high-quality primary care services, from annual checkups to chronic condition management. It’s everything you need to stay healthy and supported, with a trusted medical team by your side.

Our foundation tier gives you access to the full range of high-quality primary care services you expect from a trusted medical provider, including those listed aside.

Many of these services are typically not reimbursed by insurance but are supported through the membership model.

  • Preventive care, annual checkups, and routine physical exams

  • Management of chronic conditions (such as diabetes, hypertension, and asthma)

  • Sick visits and acute care for common illnesses or injuries

  • Health screenings and diagnostic testing

  • Immunizations and vaccinations

  • On-site procedures and lab services

  • After-hours urgent call coverage
  • Priority access to affiliated Urgent Care sites

Enhanced Membership

Our Enhanced Membership Package enhances your care with added convenience, coordination, and wellness-focused services that go beyond what insurance covers. With dedicated support and a more proactive approach to prevention, you’ll experience a higher level of personal attention and peace of mind.

For patients who want added support beyond standard medical care, our Enhanced Membership Package offers essential services that insurance does not cover but patients value most.

Enhanced members receive everything in the Basic tier plus the services listed aside.

This level is ideal for patients who want more flexibility, responsiveness, and convenience.

  • Dedicated Referral Coordinator – Faster, smoother specialist referrals handled directly by our team

  • Out-of-State Care Coordination – Help finding trusted medical providers when traveling, plus prescription support when appropriate

  • Dedicated Nurse Coordinator – Access to a registered nurse during business hours for clinical questions and guidance

  • Enhanced Comprehensive Medical Exams – Expanded wellness-focused visits with extra attention to nutrition, exercise, body composition, and prevention beyond what insurance typically covers

  • Extended annual exam
  • Direct portal messaging to your provider
  • Online scheduling and document uploads

  • $120 per year (billed annually)

  • $70 every 6 months (billed semi-annually)

Concierge Membership

For patients who want to maximize their health, vitality, and long-term wellness, our Concierge Membership delivers the most advanced level of care. In addition to everything in the Basic and Enhanced Memberships, this tier provides direct communications access to providers on demand along with cutting-edge specialty services designed to help you look, feel, and perform at your best.

Our Concierge Membership is more than healthcare — it’s a complete wellness partnership designed to help you live healthier, longer, and stronger. By combining advanced medical innovation with highly personalized care, this program gives you exclusive access to services that go far beyond traditional primary care. By having direct communication with your provider whenever you need them, you’ll have every resource at your fingertips to optimize your health and performance. If you want the convenience, expertise, and attention of a concierge-style medical experience, the Concierge Medicine experience delivers unmatched value and peace of mind.

Concierge members enjoy all Enhanced benefits plus:

  • Private, direct physician contact line (for non-urgent concerns)
  • Four free telemedicine visits per year
  • Personalized, continuous physician access for coordination and wellness
    This tier offers the highest level of convenience and access.

  • $500/year or $300 every 6 months

Will I be denied care if I do not pay for a membership?

Absolutely not. Every patient—regardless of membership level—continues to have access to all insurance-covered medical services, including annual physicals, follow-ups, and sick visits.

For patients who are unable to pay, the Basic Membership ($0/year) provides full access to all covered services plus additional benefits such as after-hours urgent coverage, referral coordination, and access to ancillary services.

We are able to sustain this level of care only because of patients who choose to participate in the Enhanced and Concierge membership tiers. Their support helps us maintain staffing, technology, and communication systems that benefit everyone in the practice.

Why is the practice moving to a membership model?

The cost of operating a medical practice has risen dramatically, while insurance reimbursement has remained nearly flat for over a decade. Much of the time and services patients value most—quick response times, coordination of care, medication management, and direct communication—are not reimbursed by insurance.

The membership model allows us to maintain these high standards of service, invest in staff and technology, and ensure long-term stability of the practice.

How was the membership fee determined?

The fee reflects the real cost of providing the non-covered services that make your care efficient, personal, and responsive—things that insurance doesn’t pay for but patients expect. The goal is sustainability, not profit. We created several tiers, including a $0 hardship option, to ensure that every patient can remain part of the practice.

What is included in the Basic (Free) Membership?

The Basic Membership covers all standard insurance-covered services—annual exams, follow-ups, sick visits, and more—plus additional support services such as:

Can I change my membership level later?

Yes. You can upgrade or downgrade your membership at any time. If your financial situation changes, you may request or rescind the hardship waiver as needed.

Does insurance cover the membership fee?

No. The membership fee covers non-covered administrative and convenience services—not medical treatment—so insurance cannot be billed for it. All medical care provided during visits remains covered and billed through your insurance as usual.

Will my insurance benefits or physician relationship change?

No. Your insurance plan, coverage, and relationship with your physician remain exactly the same. The membership simply helps ensure your doctor and care team can continue to deliver timely, coordinated, and personal service despite declining reimbursements.

How does this benefit me as a patient?

The membership program ensures your care team remains accessible, efficient, and focused on prevention and wellness rather than volume. It supports:
